We are currently accepting new patients. Anyone wishing to register with the practice should call in person to the reception desk at our Moorends Branch or alternatively, download and print off the documents located at the bottom of this page. Please complete the forms and bring them to our Moorends branch where the receptionist will be happy to guide you through the process. Please bring photographic proof of identity eg:

  • Passport
  • Drivers Licence
  • Bus Pass
  • Birth/Adoption
  • Certificate

and a Utility Bill (dated in the last 3 months) as proof of address eg:

  • Electric/gas bill
  • Bank Statement
  • Council Tax bill
  • Letting Agreement
  • Benefits Agency/Court
  • Letter
